Applying for the Yellow Vaccine Passport

Today I’m at the Department of Disease Control to apply for my yellow vaccine passport which I will use for international travel. I’m applying at the office in Nonthaburi, but you can also apply at your local health office or provincial hospital. This THREAD gives you the steps.

[2/6] I made an appointment to apply for the ‘COVID-19 Certificate of Vaccination’ from the website of the Division of Communicable Diseases: vpassport.ddc.moph.go.th. But please note, after 30 October, you must use the Mor Prom app to make an appointment.

[3/6] My appointment was at the Department of Disease Control in Nonthaburi (Map: goo.gl/maps/KdeCViUtA…). You need to go to Building 5. The security guards told me where to go before I asked. At the lobby entrance, you pick up a queue number and wait to be called.

[4/6] All you need to show them are signed photocopies of your vaccine certificate and passport. They didn’t ask to see my originals, a flight ticket, or my appointment number. You can photocopy your documents here. No passport photo is need for the vaccine passport.

[5/6] Once they have checked your documents, you take the lift up to the 6th floor. Here you must take another queue number. A short while later, my number was called and I submitted the two photocopied documents. There was no form to fill out. I was then told to wait.

[6/6] About 30 minutes later, my name was called and I went to pick up my yellow vaccine passport. All very simple. I was in and out in less than one hour and the process only cost me 50 Baht. I’m now good for international travel.

[UPDATE 1:]  The yellow vaccine passport has no expiry date. Though obviously the “life expectancy” of the vaccine will expire. So, there is space for eight more vaccination jabs.

    Very useful summary but possibly overtaken by events.I just looked at my Mor Prom app which seems to provide now (under the heading International Certificate) for the vaccination passport to be sent by post to a Thailand address if that’s what is wanted ie without any kind of appointment/visit needed in advance, ie just uploading the required information.Is that your understanding or have I missed something?

    I don’t remember any of this nine years ago. I have the yellow booklet from my hand at a certain famous Tropical Medicine Clinic in Bangkok. The MD there then told me that the Yellow Fever vaccination does more harm than good and he suggested that he stamp my booklet w/o actually innoculating me. He said that the prevention was worse than the disease. Has medical opinion changed?

        the mor prom app requires the id card number for registration but foreigners do not have an id card. Who has to release the 13 numbers?

          If you received any vaccinations, the certificate has a registration number. Use that as your id card #.

    Im a reader of your news and thank to you. I have fully vaccinated in my country and lives in Thailand now. I heard that I cannot make the vaccine passort. If it is true, is it impossible to go overseas (not my mother country) and come back to Thailand without quarantine?
    Please let me know. Thank you.

    I’m trying to get an appointment on the application but it keeps saying ‘error’. Can I get it done at the airport on my flight day or just walk-in the health disease building ?

    Not sure if there is any relevance or information available on this.
    Using the official app i have applied for a covid passport and the first available pick up date is in December? The point of getting this was to travel somewhere before then. Any clues on the flexibility of the date?

    Hai Richard, I already had my second vaccine but when i tried to register for mu 3rd vaccine it said the ministry of public health didn’t have any record of my vaccine. What should i do to have my 3rd vaccine? Should i get my vaccine passport first? Thank you in advance ka🙏🏻

    There is nothing new with this people.
    When i got my first vaccine, I brought my international vaccination yellow book. It is curious that at the hospital nobody knew what it is, and they just gave me a “certificate”; no stamp in my book.
    Now they created a copy of this book, but “Made in Thailand”. 5555

    I got my Yellow Passport today. Thanks to Richard’s detailed post. I would like to add one more thing here. You can go to the address given above in this post without an appointment. They accept people without an appointment at 11.00 and 15.00. Just be sure to go there an hour before as the queue was really long.

    I see on the app that they have 3 options: self pickup, postal delivery and email (e-cert) only. But only the self pickup can be ticked. Any idea when they will open the other services? Thanks

      When I got my second jab, the hospital gave me a vaccination certificate and that piece of paper has my new Thai ID number.

        A friend told me you know where to get Pfizer or AstraZeneca vaccines in thailand ? Could you please let me know as I cannot fly to uk with sinopharm

      If you received either the paper or Mor Prom “Thailand Certificate of Covid-19 vaccination” there will be a registration number. It is the 5th row down on the left opposite your passport number.

      What is frustrating, if you allow such things to frustrate, the booster is absent from the Mor Prom app. I have to carry a photo and a piece of paper. I do not understand what genius did not include all Covid-19 vaccinations on the Mor Prom Certificate.

      “Disappointment springs from unmet expectations”. All citizens should have a right to demand continuous improvement from governments. In this case I am not a Thai citizen yet this is a pandemic and I am a citizen of the world. The Thai’s should expect no less when in a foreign land.

      Unknown but take a signer power of attorney just in case. Forms can be downloaded from the internet.

    HI,
    I am traveling to my home country this month end. I have got my vaccination certificate as well but I failed to make an appointment for yellow vaccine passport. I cannot make an appointment before my travel date on the website. Is it possible to go directly and apply for one?

      You can use the Mor Prom app. There is an option to send to an address but it is not active yet.

      The hsopital where you got your second jab should have given you a certificate that had your new Thai ID number on it.

      No, you should have your own vaccine passport from the country you were vaccinated. But I understand that those who had booster shots abroad can now update this in the yellow passport.
